how to take multiple inputs from a user as a form template and then write the inputs to a .txt file?

i want an user to fill a form which will contain multiple questions and all the inputs provided by the user should be written in .txt file.

This is far to general to be answered. Please show us, what you have tried so far an explain the open problems. Currently the best answer would be to let the user open a standard text editor to sill in the data directly.
