Neural Network (choose of the target?)

I have data set of Wisconsin Diagnostic Breast Cancer and datasets-uci-diabetes. Now i need to import data into nntoolbox i don't know how to create target dataset and input dataset (which attributes i should taken for target??? and which for input in case of fitting tool, pattern recognition tool, time series tool???), i have attached the datafile.
