how to do regression for deriving relationship when there are five inputs and one output?what is command and syntax for this?

i need to find soil liquefaction using ANN. i m applying supervised approach.target output is evaluated by empirical method.now i have to find liquefaction using ANN. how i will find the relationship between the five inputs and output so that in future i can use the relationship to get outcome for given inputs.
