Error in simulink: Subscripted assignment dimension mismatch

Error in simulink: Subscripted assignment dimension mismatch.
Model Error. Source Unknown. Reported By Simulink.
I built a model in Simulink 2015b and works perfect. For some reasons I had to shift to 2013b version. I exported model to save as 2013b version. After saving it this 2013b version model runs perfect on 2015b version of matlab but gives errors in 2013b version of matlab. Fixed a few errors until I got stuck on this error.

Hi Tayyab,
Can you please attach a simplified model (that shows the blocks you are using in your model) where the issue is reproducible?
