Tutorial for using matlab simulink UDP send/recieve with C++

Hello I am new to simulink and was interested to use the UDP send /UDP reciever simulink model. The idea is to use the input data vector for UDP send as a output of C++ program and send it over as a UDP message. While on the UDP recieve side the aim is to correctly decode the send message. Any help regarding this would be greatly appreciated.
