give an example to code lookup table step by step to enter inputs and to get outputs ?

To check: is this for MATLAB or for Simulink ?
Will the lookup always be exact, or does interpolation need to be done?
Is the lookup table one that presents exact data up to a certain number of decimal places, and then requires calculation to get the final decimal places?
