what is the problem of my ANN program and i want to reduce its error. nueral network training performance image is attached.

kindly any one explain me this
what is the problem my ANN program is having and how it can be sloved
there is a difference between targets and outputs. does it mean that training is not done properly or what.
i have also the hidden layer size but it is not giving me less error
