How to calculate the arterial wall thickness on an image?

Hi everybody!
I am a beginner in Matlab, and I do not know how I should approach this problem.
After I used the region growing to segment the common carotid artery wall, which has a plaque (stage III), I have to calculate the blockage percentage according to the wall thickness. I calculate the thickness using imtool, but how do I transform it in millimetres? An ideal wall thickness of the carotid artery is 0.7mm.
Thank you!

You should have some scale parameters
Here is idea:
human = 45 * 3/100 = 1.35 [m]
