How can make Nyquist plot from numerical results like RAO and phase?

I would like to make a Nyquist plot from a solution of my numerical model, I do not know how do that. From my model I can extract a numerical value of my Response amplitude opearator (RAO) and the phase of the response of the linear system as a function of the frequency of the incoming signal, how can I create a the Nyquist plot, start to these two results?
