Caries Identification in Dental X ray images

i attach the four images two the original images and the other two is the desired results image, i am confused how to do this because disease part varies for teeth to teeth and same intensity pixels exist in the caries and root part of dental images but i wanted to highlight or segment out only the disease part from the teeth x-ray image. kindly help me on this.
