Error Index Exceeds matrix dimension - Multi ANN and Multi output ANN optimization using PSO?

I would like to optimize the 3 input and 2 output using PSO. It is working for single output ANN. It is not working for multi-output ANN. How to solve the error "Index exceeds matrix dimensions"? How to set weights to ANN?
