where to copy information in 'file installation key' dialog box on license centre web page?

I am installing matlab2014, I have that set up with me ,I choose the option of installation without internet then option 'I have license key' when I entered license key it gave error.Then i choose option to get license key, I copied data from 'file installation key' dialog box. I visited the web page of license center of your's but where to copy the host ID, release & windows user name?

If you're having difficulty using the License Center, I recommend you use the Contact Us link in the upper-right corner of this page to contact Technical Support. They should be able to help you determine where on the License Center you need to enter your information.
