How can I create something exactly like this? (Matlab on web)

I want to create a very simple webpage where I can enter a few inputs and click a button to get a surface:

That is exactly what I want. I would like help with this. I have a .m and a .fig file of something that looks like this, but not sure how to bring it to html.
