Is it possible to allow user to add a note to a GUI?

I am in the middle of making a data mining GUI and thinking of making a button in my GUI that says "report an error", when a user presses it, he/she can edit it with his/her name and subject and save it to the GUI, it will be nice if this message will be sent out to a person's email or simply saved in the GUI and available for whoever open the GUI afterwards.

Maybe I am making things up here but I still would like to ask you all experts' opinions on this... Thank you in advance, and I appreciate your time reading my post.
