How to model a harmonic translation

Hello Community
I'm trying to create a model that creates a harmonic (sine-wave) translation from a rotation. Basically I'm trying to model the behavior of the following system.
Creating a translation from a rotation can be easily done with the wheel and axle block. But I really don't know how to model the required behavior that is mentioned above.
