C2000 DSP data visualization in simulink

Hi ;
ı can ask something about data visualization in simulink. How to plot in simulink by data got from C2000 DSP. so how can ı read ADC port and plot simulink in real time application(My DSP is TMS320F28069M & TMS320F28379D)
